The City Council of the City of Stephenville, Texas, convened in special session on Tuesday, February 16, 1999, at 5:30 p.m., in the Council Chambers at City Hall, 298 West Washington Street, with the meeting being open to the public and notice of said meeting, giving the date, time, place and subject thereof, having been posted as prescribed by Chapter 551, Government Code, Vernon's Texas Codes Annotated, with the following members present, to wit: Mayor: Councilmembers: Absent: Others Attending: L CALL TO ORDER John Moser Barbara Terrell Marshall Shelton Joel Hillin K. H. Conatser Bea Marin Jerry B. Madkins Rusty Jergins Wm. H. (Bill) Stanley Don Davis, City Administrator Cindy L. Stafford, City Secretary Randy Thomas, City Attorney Mayor John Moser called the special meeting of the Stephenville City Council to order at 5:30 p.m. on Tuesday, April 20, 1999. Mayor Moser declared a quorum present. II. PERSONNEL COMMITTEE REPORT A. Health Insurance Council member Madkins reported that the Personnel Committee had met to review health insurance rerates and premiums for health insurance for the period from May 1, 1999 to April 30, 2000. Madkins turned his report over to City Administrator, Don Davis, who gave a more in -depth explanation of the process involved. He reminded the council that the city is self - insured. Three types of proposals were bid —one for a third party administrator and two for co- insurance (one for individual coverage above $15,000 and one for aggregate coverage for the total of all claims). The council reviewed the various options. April 20, 1999 - Minutes of Special Meeting 5 1, 1. MOTION by Jerry Madkins, second by Marshall Shelton, to accept the recommendation of the Personnel Committee and adopt Option 2, which provides for an increase of 13.65% in both the city and dependent cost. Motion carried by unanimous approval. MOTION by Jerry Madkins, second by Bea Marin, to opt out of the provisions of the Health Insurance Portability and Accountability Act. Motion carried by unanimous approval. III. ADJOURN MOTION by K. H. Conatser, second by Joel Hillin, to adjourn the meeting. WASHINGTON, STEPHENVILLE, TEXAS 76401 (254/918- 1220), a Texas political subdivision employees' uniform group benefits plan administered by the Texas Municipal League Group Benefits Risk Pool, 1821 Rutherford Lane, Suite 300, Austin, Texas 78754 -5151 (512/719- 6500), pursuant to 42 U.S.C. § 300gg -21, hereby elects to be exempt for the plan year beginning May 1, 1999 through April 30, 2000 from the following requirements of the Health Insurance Portability and Accountability Act of 1996: {A check in the box indicates entity's decision to be exempt} Limitations on preexisting condition exclusion periods ® Special enrollment periods for individuals (and dependents) losing other coverage. ® Prohibitions against discriminationagainst individualparticipants and beneficiaries based on health status. ®{ Standards relating to benefits for mothers and newborns ial Parity in the application of certain limits to mental health benefits. The City of Stephenville Health Benefit Plan is a self-funded program and does not include health insurance coverage. This election is being made at a duly called open meeting of the City of Stephenville on April 6, 1999, at 298 W. Washington, Stephenville, Texas. The attached "Notice to Plan Participants Regarding the City of Stephenville Election Under 42 U.S.C. § 300gg -21 "will be provided to all Plan participants by being prominently printed in the Plan's summary plan document or distributed to each employee prior to the beginning of the plan year.
