The Employees Benefit Trust of the City of Stephenville, Texas, convened on April 7, 2015, at
7:44 p.m., In the Chambers at City Hall, 298 West Washington Street, for rho ,mirpose of the
annual meeting, with the meeting being open to the public and notice of said meeting, giving the date,
time, place and subject thereof, having been posted as prescribed by Chapter 551, Government Code,
Vernon's Texas Codes Annotated, with the following members present, to wit:

Call Meeting to Order. President Weldon called the meeting to order at 7:44 p.m.
II. Approve Minutes from the April 1, 2014 Employee Benefits Trust Meeting. MOTION by
Doug Svien, second by Russ McDanel, to approve the minutes of the April 1, 2014
Employee Benefits Trust Meeting. MOTION CARRIED.
III. Consider Action Awarding a Contract for Employee Health Insurance. MOTION by Doug
Svien, second Brady Pendleton, to award a contract to United Health Care for employee
health insurance. MOTION CARRIED.
IV. Consider Action Awarding a Contract for Employee Dental Insurance. MOTION by
Doug Svien, second by Russ McDanel, to award a contract to United Health Care for
employee dental insurance. MOTION CARRIED by unanimous vote.
V. Consider Action Awarding a Contract for Employee Voluntary Life and Long -Term
Disability. MOTION by Doug Svien, second by Sherry Zachery, awarding a contract to
Financial Benefit Services for employee voluntary life and long-term disability. MOTION
CARRIED.
VI. Adjourn. The meeting was adjourned at 7:51 p.m.
